$100 one-time enrollment fee per person or family
Individual Plans
Children 0-19 years $45/month (with parent membership)
Children 0-19 years $75/month (without parent membership)
Adults 20-50 years $100/month
Adults 51+ years $125/month
Family Plans
2 adults=$175
2 or more children WITH a parent
$45 per child
Each additional child is $15
2 or more children WITHOUT a parent on the plan
$75/month/child for first 2 children
Each additional child $15
Max $250/family
