Membership Pricing

Individual Plans

  • $100 enrollment fee

  • Children 0-19 years with parent enrolled $50/month

  • Children 0-19 years without parent enrolled $75/month

  • Adults 20+ $100/month

  • Medically complex patients $125 (provider discretion)

Family Plans

  • $100 enrollment fee per family

  • 2 adults=$175/month

  • 2 or more children WITH a parent enrolled

    • $40 per child/month

    • Each additional child is $15/month

  • 2 or more children WITHOUT a parent on the plan

    • $75 per child for first 2 children/month

    • Each additional child $15/month

  • Family maximum $250/month
